Job summary

Pathways Home Health & Hospice is seeking a Registered Nurse (RN) Case Manager to join our hospice team in Sunnyvale, CA. This full-time role involves admitting patients, assessing needs, and coordinating care with a multidisciplinary team.

You will document observations, develop care plans, and participate in quality improvement activities. A valid California RN license and CPR certification are required, with a sign-on bonus of $9,000 and a rotating weekend/holiday schedule.

Qualifications

• Current California RN license.
• Minimum of one year of nursing experience.
• Current CPR Certificate.
• Current California driver's license and automobile insurance.

Responsibilities

• Performs an initial, comprehensive assessment.
• Documents observations, clinical findings, problems, skilled interventions, goals and discharge plans.
• Initiates and regularly re-evaluates and revises plan of care with the Clinical Team Manager.
• Assesses the need for services of other team members (PT, OT, ST, MSW, HHA).
• Provides and documents skilled care on all visits and teaches patient/family.
• Obtains and documents physician orders.
• Coordinates care with the Clinical Team Manager and other home care team members.
• Submits weekly visit schedule of assigned patients.
