Job summary

Moments Hospice in Minnesota is seeking an RN Case Manager to deliver compassionate, patient-centered care across the continuum. You will conduct initial assessments, refine care plans, provide hands-on nursing care, and coordinate patient care.

Qualifications

• Must be a licensed RN in the state you work.
• Proficiency with EMR software.
• Strong planning, organizing, and communication skills.
• Ability to solve problems autonomously while meeting responsibilities.
• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Conduct initial client assessments and refine care plans.
• Provide hands-on nursing care and patient education.
• Maintain open lines of communication with patients, families, and the healthcare team.
• Manage patient care coordination for your caseload.
• Participate in interdisciplinary team meetings to ensure comprehensive care planning.
