Job summary

Blue Ridge Care is seeking a compassionate Registered Nurse (RN) Case Manager to join our hospice team in Frederick/Clarke County. You will coordinate individualized care plans and work with an interdisciplinary team to improve patients' and families' quality of life.

The ideal candidate will have Virginia RN license, CPR, at least two years of nursing experience (hospice or home health preferred), and strong clinical judgment.

Qualifications

• Current Virginia RN license (multi-state or single-state).
• CPR certification and >=2 years of nursing experience (hospice or home health preferred).
• Strong clinical judgment, communication, and organizational skills.
• Reliable transportation and valid driver's license.

Responsibilities

• Assessing patient and family needs and developing personalized care plans.
• Delivering skilled, compassionate nursing care and symptom management.
• Administering medications and coordinating care across the healthcare team.
• Educating and supporting families to promote comfort and peace of mind.
• Leading and guiding hospice aides to ensure consistent, compassionate care.
• Maintaining accurate documentation and participating in ongoing education.
