Job summary

Amedisys is seeking a dedicated RN Case Manager to coordinate patient care in the Harwich and Brewster area, with orientation at our Hyannis care center. This full-time role offers a $7,500 sign-on bonus and competitive pay, plus benefits including health plans and a 401(k).

You’ll work with the care team to develop and implement care plans, supporting patients at home. Required is at least one year of nursing experience, an active RN license, CPR certification, and a valid driver's license.

Responsibilities

• Assess patients and collaborate with care team to plan care.
• Refer to hospice disciplines as needed or document rationale.
• Maintain individualized patient care progress records accurately.
• Record pain/symptom management changes/outcomes.
• Educate patients, families, and caregivers on care techniques.
• Communicate with clinical manager and care team regularly.
• Perform on-call duties as assigned.
• Other duties as needed.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
