Job summary

Amedisys is hiring an RN Case Manager for the Greater Framingham area, with orientation in Marlborough. This full-time Day Shift role includes a $10,000 sign-on bonus and a compensation range of $96,000–$105,000 per year. Mileage reimbursement and vehicle fleet programs may apply.

Responsibilities include patient assessments, care planning, referrals, documentation, and caregiver education, with regular team communication and on-call duties as needed.

Qualifications

• One year of nursing experience.
• Current RN license for assigned states.
• Current CPR certification.
• Valid driver's license, reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
