Job summary

Amedisys is seeking an RN Case Manager for full-time day shifts to support patients at home. You will perform patient assessments, develop care plans with the team, and coordinate hospice services to ensure quality at-home care.

Qualifications include an active RN license in the state(s) of assignment and CPR certification, with a minimum of one year of nursing experience. A valid driver's license is required.

Qualifications

• Current RN license for the state(s) assigned.
• Valid CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess patients and collaborate with care team to develop and implement a care plan.
• Refer to other hospice disciplines as needed and document rationale.
• Maintain individualized patient care progress records with accuracy and timeliness.
• Record pain/symptom management changes and outcomes.
• Educate patients, families, and caregivers on care techniques.
• Communicate regularly with clinical manager and care team.
• Perform on-call responsibilities as assigned.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
