Amedisys is hiring an experienced RN Case Manager to join our home health and hospice team serving Cambria, Somerset, and Indiana Counties. You will perform patient assessments, develop and implement care plans, and collaborate with the care team to help patients heal at home.

We offer attractive pay, a $5,000 sign-on bonus, choice of medical plans with telehealth, PTO, wellness rewards, and a 401(k) with company match. Valid RN license, CPR, and a driver's license required.

Qualifikationen

• One year of nursing experience.
• Current RN license valid in assigned state(s).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.
