Job summary

Assisted is seeking a full-time or per diem RN Case Manager to join our Hospice Team at the Ventura branch. Spanish speaking is preferred but not required. We offer a rewarding benefits package designed to support health, financial well-being, and career growth.

Qualifications

• Graduate of an approved school of professional nursing.
• Current, active license in the state of California Board of Nursing.
• Current CPR, health clearance, TB clearance.

Responsibilities

• Have flexible hours and working close to home (reduce your commute!).
• Complete initial and on-going comprehensive assessments of the patient’s needs
• Provide service and /or treatments requiring substantial and specialized nursing skills
• Initiate appropriate preventative and rehabilitative nursing procedures
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs
