Job summary

Harbor Healthcare System is seeking a full-time Registered Nurse (RN) Case Manager in the Austin area. The RN will work in an outpatient setting, coordinating admissions, assessments, discharges and pronouncements with an interdisciplinary team.

Qualified candidates hold Texas RN licensure, with 1+ years of RN experience preferred. Hospice or Home Health exposure is a plus, and travel with reliable transportation is required. Strong communication and clean background are essential.

Qualifications

• Licensed RN in the State of Texas is required.
• 1+ years RN experience preferred.
• Experience with Hospice or Home Health preferred.
• Effective written and verbal communication.
• Ability to travel, reliable transportation required.
• Clean background, drug screen, and driving record.

Responsibilities

• Admit and coordinate care for each patient and family.
• Perform assessments and pronouncements; manage discharges.
• Collaborate with interdisciplinary care team to ensure appropriate care.
