Job summary

Amedisys is seeking an RN case manager to provide comprehensive home health care in Cambria, Somerset, and Indiana Counties. You will assess patients, develop care plans, coordinate with the care team, and educate families to support healing at home.

This role includes on-call responsibilities and a competitive sign-on bonus. Qualified candidates hold an active RN license, CPR certification, and at least one year of nursing experience, with a valid driver’s license and reliable transportation.

Qualifications

• One year of nursing experience required.
• Current RN license valid for the assigned states.
• CPR certification required.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to hospice care disciplines as needed or documents rationale.
• Maintains individualized patient care progress records accurately and timely.
• Records pain and symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate care techniques.
• Communicates regularly with clinical manager and care team.
• Performs on-call responsibilities as assigned.

• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Teaches patients, families, and caregivers appropriate patient care techniques.
• Regularly communicates with clinical manager and care team.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Other duties as assigned.

Qualifications

• One year of nursing experience.
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
