Job summary

PASCO/SW Hospice seeks a dedicated Hospice RN Case Manager to provide in-home nursing care across Colorado. You will assess patients, build care plans, and guide families with compassion in life’s challenging moments.

The role requires an active Colorado RN license, 2+ years’ nursing experience (hospice/home health preferred), and the ability to work independently. A driver’s license and insured vehicle are essential.

Qualifications

• Active Colorado RN license.
• Minimum of 2 years nursing experience; hospice/home health preferred.
• Homecare Homebase (HCHB) experience preferred, not required.
• Valid driver’s license and insured vehicle.
• Strong clinical skills and patient-centered care.
• Ability to work independently.

Responsibilities

• Complete comprehensive patient/family assessments to determine Hospice needs.
• Develop and implement individualized care plans and revise as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, meds, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities.
• Collaborate with an interdisciplinary team to maintain high standards.
• Participate in on-call rotation as needed.
