Job summary

Atrio Home Care, a partnership of Holland Home and Resthaven, seeks an RN Case Manager for home visits in Holland/Lakeshore. You will provide direct care to clients in Ottawa and Allegan Counties, coordinating care with physicians and the care team.

The role requires an active MI RN license, with BSN preferred, and 1–2 years of acute or skilled nursing experience. A valid MI driver’s license and reliable transportation are essential for home visits.

Qualifications

• Licensed RN in Michigan; BSN preferred.
• Must have valid Michigan driver's license and auto insurance.
• 1–2 years of acute care or skilled nursing experience; home health preferred.

Responsibilities

• Perform initial and comprehensive assessments and deliver compassionate care to clients.
• Collaborate with physicians and care team to develop a Plan of Care.
• Promote client and family independence through counseling and education.
• Coordinate client discharge planning.
• Identify barriers to outcomes and evaluate care results.
• Prepare clinical documentation and progress notes in a timely manner using EMR on iPads.
• Participate in on-call rotation.
