Qualifikationen

Aufgaben

• Perform initial and comprehensive assessments and deliver compassionate care.
• Collaborate with physicians and other care team members to develop a Plan of Care.
• Promotes client and family/caregiver independence through counseling and education.
• Identify barriers to desired outcomes and evaluate outcomes of care.
• Prepare clinical documentation, progress notes using iPads for the EMR system.
• Participates in on‑call rotation.
