Job summary

Deer Meadows is seeking a dedicated RN Case Manager to provide in-home nursing care and comprehensive case management for patients in their homes. The role focuses on coordinating care with families and healthcare providers to ensure seamless transitions and optimal outcomes.

Ideal candidates will hold PA RN licensure with home health or case management experience, strong clinical skills, and the ability to work independently and as part of a team. A valid driver’s license is required.

Qualifications

• Current licensure as a Registered Nurse in the State of Pennsylvania.
• Experience in home health care and case management.
• Strong clinical and case management skills.
• Excellent communication, interpersonal and organizational skills.
• Ability to work independently and as part of a team.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess patients in their homes and create individualized care plans.
• Coordinate with patients, families, and providers to ensure continuity of care.
• Provide skilled nursing care including wound care, med management, and education.
• Conduct regular home visits to monitor progress and adjust plans as needed.
• Maintain accurate patient records and reports.
• Participate in quality improvement initiatives and collaborate with teams.
