Job summary

Deer Meadows Home Health is seeking a compassionate RN to serve as Home Health Admissions RN. You will perform initial assessments, create and monitor home care plans, and coordinate with an interdisciplinary team to ensure high-quality, patient-centered care in the home setting.

The role requires PA nursing licensure, home health experience, and strong communication skills. A $5,000 sign-on bonus is offered along with competitive compensation.

Qualifications

• Current RN license in Pennsylvania.
• Experience in home health and Oasis assessments.
• Excellent communication and problem-solving skills.

Responsibilities

• Conduct initial assessments and develop care plans for patients in their homes.
• Implement and monitor treatment plans, ensuring continuity of care with patients and families.
• Collaborate with physicians and therapists to update patient care plans.
• Educate patients and caregivers on disease management and rehabilitation strategies.
• Identify and address patient safety concerns following evidence-based guidelines.
• Document patient progress and care activities accurately and timely.
• Maintain a positive, respectful environment for patients, families, and colleagues.
