Job summary

Rochester Regional Health Home Care is seeking an RN Case Manager to deliver comprehensive nursing care to assigned patients. The role includes assessing needs, developing and evaluating a Plan of Care, coordinating interdisciplinary services, and supervising Private Duty Nurses as applicable.

Full-time position based in Monroe County - West Team with a Monday-Friday 8:00am-4:00pm schedule. Features a signing bonus up to $20,000, mileage reimbursement, shift differentials, overtime

Qualifications

Nursing diploma or associate degree required.
Registered Nurse license in NYS required.
Valid NYS driver’s license required.
BSN preferred.

Responsibilities

Assess patient and family needs and develop a Plan of Care.
Coordinate interdisciplinary services and supervise PDN as applicable.
Manage and coordinate care for an assigned caseload.
Document clinical notes and assessments in the EMR.
Travel to patient locations as needed.
