Job summary

Rochester Regional Health is seeking an RN Case Manager for Home Care in Monroe County. You will assess patient needs, develop and evaluate a Plan of Care, and coordinate interdisciplinary services. The role includes supervising PDNs and ensuring timely, quality patient care.

40 hours per week, Monday-Friday, with a signing bonus up to $20,000. Mileage reimbursement, paid drive time, and tuition assistance are offered as part of a comprehensive benefits package.

Qualifications

• Diploma or an Associate’s Degree in Nursing is required.
• Active RN license in New York State is required.
• A valid New York State driver’s license is required to perform home visits.

Responsibilities

• Assess patient needs and develop a Plan of Care.
• Coordinate services across the care team to ensure timely delivery.
• Supervise home health staff and PDN as applicable.
• Document care and communication per agency standards.
• Manage and prioritize caseload to meet care goals.

Education
