Job summary

Rochester Regional Health Home Care in Newark is seeking an RN Case Manager to deliver comprehensive nursing care and coordinate an interdisciplinary Plan of Care.

Responsibilities include patient assessment, care coordination, discharge planning, and EMR documentation, with a typical Monday–Friday schedule and a signing bonus up to $20,000. Travel to patient locations is required, and tuition assistance programs are available.

Qualifications

• Nursing license required (RN)
• Diploma or Associate's degree in nursing required
• BSN preferred for advancement or leadership roles

Responsibilities

• Assess patient and family needs and develop a Plan of Care
• Coordinate services and supervise PDN as applicable
• Manage and coordinate patient care for an assigned caseload
• Document care and assessments in EMR per policy
• Participate in discharge planning and care conferences
• Support on-call and weekend/holiday schedule
