Job summary

Carle Health is seeking a Home Health Registered Nurse to travel to patient homes and provide skilled nursing care across the service area. The role emphasizes coordinating care, creating care plans, and educating patients and families while adhering to nursing standards.

The ideal candidate will have an active RN license, BLS within 30 days, and a nursing diploma. Sign-on and relocation bonuses are available for eligible external candidates who meet experience requirements.

Qualifications

• RN licensure (Registered Nurse) as required.
• BLS within 30 days - American Heart Association.
• Auto Insurance proof and Driver's License as required.

Responsibilities

• Demonstrates understanding of Medicare Home Care benefits including covered services, recertifications, transfers, and non-recertifications.
• Follows regulatory documentation requirements and standards.
• Performs case management: assessment, coordination, planning, monitoring, implementation, evaluation.
• Communicates with clients, caregivers and families to plan care and arrange services.
• Completes admission documentation timely and accurately.
• Performs home visits notes, phone records, and transmits documentation per policy.
• Reviews physician orders and ensures timely transmission without alterations.
