Job summary

Amedisys is seeking a registered nurse case manager to deliver high-quality home health services in the Manchester area. You will assess patients, develop care plans with the care team and coordinate referrals to appropriate disciplines.

You will supervise LPNs and HHAs, complete timely documentation, and communicate progress to the clinical manager. Amedisys offers competitive pay, benefits, and growth opportunities in a supportive environment.

Qualifications

• 1+ year of RN clinical experience; approval by VP Clinical if less.
• Current RN license for assigned states.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures; teaches self-care techniques.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
