Job summary

Amedisys is seeking a Registered Nurse Case Manager in Medford, NY to join our home health team. You will assess patients, coordinate care, and guide families while ensuring high-quality, compassionate service in the comfort of the patient’s home.

Qualifications

• One (1+) year of clinical experience as a Registered Nurse (RN).
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities as assigned.
• Participates in continuing education.
