Job summary

Amedisys is seeking a registered nurse case manager in Tucson, AZ to coordinate home health and hospice services. You will assess patients, develop and implement care plans, and collaborate with the care team to promote independence and healing at home.

This role requires at least 1 year of RN clinical experience, a current RN license for the state, CPR certification, and a valid driver's license with reliable transportation. Amedisys offers competitive pay and generous benefits.

Qualifications

• 1+ year of clinical experience as an RN.
• Active RN license for the assigned state(s).
• Current CPR certification.
• Valid driver’s license with reliable transportation and liability insurance.
• RN intern program if less than 6 months experience.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitation measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
