Job summary

Amedisys Home Health is seeking a Registered Nurse Case Manager to assess patients, coordinate care plans, and collaborate with the care team to ensure quality at-home treatment. You will supervise LPNs and HHAs while maintaining thorough documentation and ongoing communication with the clinical manager.

The role emphasizes teaching patients self-care techniques, on-call coverage, and participation in ongoing professional development.

Qualifications

• One year or more of clinical RN experience; less than 1 year may require VP approval.
• Current RN license for the state(s) assigned.
• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.
• If <6 months RN experience, participate in RN Intern program.

Responsibilities

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient’s needs or documents rationale for not doing so.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and services to patients/families as assigned.
• Participates in clinical development and continuing education programs.
• Other duties as assigned.
