Job summary

ATEX Hospice - Valley in McAllen, TX seeks a registered nurse for full-time home health and hospice care. The role focuses on planning, directing home care services, and coordinating interdisciplinary teamwork to meet patient and family needs in the home setting.

The ideal candidate holds an active RN license in Texas, CPR certification, and 1–2 years of acute care experience, with home health/hospice preferred. Strong communication and organizational skills are required; driving is expected.

Qualifications

• Must hold a current and valid nursing licensure in the state employed.
• Bachelor’s degree with one year of home health and/or hospice care experience preferred.
• One to two years of recent acute care experience in an institutional setting.
• Must hold a current and valid CPR certification.

Responsibilities

• Performs initial and ongoing OASIS assessments to determine home care needs.
• Develops and revises the plan of care with patient and family based on nursing diagnosis.
• Provides patient and family education on disease process, self-care techniques, end-of-life care, and care planning.
• Acts as Case Manager and coordinates patient care with the IDG/T to ensure continuity of care.
