Job summary

ATEX Healthcare Home Health is seeking a dedicated Registered Nurse to lead home health and hospice nursing activities. You will perform OASIS assessments, develop and update care plans, administer medications, and educate patients and families.

Responsibilities include coordinating interdisciplinary care, discharge planning, on-call coverage, and supervising aides to ensure quality and continuity of services in patients’ homes and communities.

Qualifications

• Must hold a current and valid nursing licensure in the state employed.
• Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.
• Bachelor’s degree with one year of home health and/or hospice care experience preferred.
• One to two years of recent acute care experience in an institutional setting.
• Must hold a current CPR certification.
• Must be a licensed driver with insured vehicle in good condition.

Responsibilities

• Completes initial and ongoing OASIS assessments of patients and families to determine home care needs.
• Develops and maintains plan of care with goals based on nursing diagnoses and patient/family input.
• Administers medications and treatments as prescribed; provides patient and family education.
• Communicates with physicians and care team about changes in patient status and orders.
• Performs discharge planning and coordinates care across the Interdisciplinary Group/Team.
• Supervises HH and Hospice aides, provides direction every fourteen days.
• Participates in on‑call duties and ensures equipment and services are available.
