Job summary

Qualifications

• Bachelor’s degree in Nursing required.
• CPR certification required.
• Current RN licensure in the employing state.
• 1 year home health and/or hospice care preferred.
• 1–2 years acute care experience in an institutional setting.

Responsibilities

• Perform initial and ongoing OASIS assessments and develop plans of care.
• Provide comprehensive physical assessments and medical histories.
• Reassess patient nursing needs and revise plans of care as needed.
• Administer medications and treatments as prescribed.
• Counsel patients and families on care needs and end-of-life considerations.
• Coordinate care with the IDG/T and educate staff as needed.
• Identify discharge planning needs and coordinate transitions of care.
• Act as Case Manager for assigned caseload when directed.
