Amedisys, part of the Optum family, is seeking an RN Case Manager to guide patients and families through the care continuum. You will assess needs, coordinate care, support discharge planning, and manage outpatient care under supervision by an RN or MD, ensuring appropriate level of care and education.

This role includes on-call responsibilities, collaboration with a Care Advocate and Social Worker, and ongoing professional development opportunities within a supportive environment.

Competenze

• Current RN license, specific to the state you are assigned to work.
• 1+ year of clinical experience as a Registered Nurse (RN).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Mansioni

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities and participates in continuing education.
