Job summary

UnitedHealth Group's Amedisys division is hiring a Nurse Case Manager to guide patients and families through the care continuum. Under supervision of an RN or MD, you will assess needs, coordinate care, support discharge planning, and manage outpatient care, while educating and coaching patients and collaborating with Care Advocates and Social Workers.

The role emphasizes on-call responsibilities, staff education, and ongoing clinical development, with compensation combining per-visit payments

Qualifications

• Current RN license, state(s) you are assigned to work.
• 1+ year of clinical RN experience.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and collaborate to develop and implement a plan of care.
• Make referrals to other disciplines as indicated and document rationale.
• Promote patient health and independence through teaching and rehabilitative measures.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and services to patients/families as assigned.
• Participate in clinical development and continuing education programs.
• Other duties as assigned.
