Qualifications

• Current RN license required.
• CPR certification required.
• Strong assessment and critical thinking skills.
• Excellent communication and care coordination abilities.
• Ability to work independently and manage multiple priorities.
• Commitment to patient‑centered care.

Responsibilities

• Assess patient needs and develop individualized plans of care.
• Coordinate services with physicians, therapists, caregivers, and other healthcare professionals.
• Manage and monitor patient caseloads.
• Provide education and support to patients and families.
• Ensure quality outcomes through ongoing assessments and care planning.
• Promote patient comfort, independence, and well-being.
• Deliver compassionate care in Home Health and Hospice services.
