Amedisys, part of the Optum family, is looking for a Nurse Case Manager to guide patients and families through the care continuum in the home health setting.

Under RN or MD supervision, you assess needs, coordinate care, support discharge planning, and collaborate with care teams to ensure appropriate care levels and outcomes. Strong focus on education, coaching, and ongoing professional development.

Qualifications

• Current RN license valid in the assigned state(s).
• CPR certification required.
• Minimum 1 year of clinical RN experience.
• Valid driver's license and reliable transportation.

Responsabilités

• Perform patient assessments and collaborate on the care plan.
• Coordinate referrals and support discharge planning.
• Educate patients on self-care and rehabilitation.
• Supervise LPNs and HHAs as assigned.
• Document timely and accurately and communicate progress.
