Amedisys is seeking a registered nurse case manager in Tallahassee, FL. This full-time role offers competitive pay and benefits, with responsibilities focused on patient assessments, care planning, and collaboration with the care team to support patients at home.

You will supervise support staff, document promptly, and participate in ongoing clinical development while providing exceptional home-based care to patients.

Competenze

• At least 1 year of clinical RN experience.
• Current RN license in applicable state(s).
• CPR certification required.
• Valid driver's license and reliable transportation.

Mansioni

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by the patient's needs and documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Regularly communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on-call responsibilities for patients/families as assigned.
• Participates in clinical development and continuing education.
