Job summary

Infiniteus Home Healthcare P.C., dba Cura Care Connect Staffing, is seeking a compassionate RN Case Manager to oversee patient care coordination and ensure regulatory compliance across our DFW service areas. You will conduct assessments, develop care plans, supervise clinical documentation, and collaborate with interdisciplinary teams to deliver patient-centered home health.

Active Texas RN license required.
Experience in home-health nursing or clinical case management preferred.
Strong understanding of Texas home-health regulations and documentation standards.
Reliable transportation.
Excellent communication, leadership and organizational skills.
Ability to manage multiple cases and support staff.

Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans.
Provide skilled nursing care in accordance with physician orders and regulatory standards.
Monitor patient progress and adjust care plans as needed.
Coordinate care with physicians, therapists, aides, and interdisciplinary team members.
Review clinical documentation for accuracy, completeness, and compliance.
Educate patients and families on care plans, medications, and disease management.
Support onboarding, training, and performance development for clinical staff.
Ensure readiness for audits, surveys, and regulatory reviews.
Maintain compliance with state and federal home-health regulations.
Communicate updates to the Clinical Manager and Administrator.
