Job summary

UnityPoint Health - Home Health is seeking an experienced RN Case Manager to deliver one-on-one nursing care with total autonomy over the patient care plan in patients' homes. You will manage admissions, create and adjust care plans, and discharge coordination, ensuring high-quality outcomes and exceptional patient experience.

Qualifications

• Graduate of State Board approved program for Registered Nurses.
• Valid RN license in state(s) where providing care.
• Valid driver's license with automobile insurance.
• Mandatory Reporter: Child & Dependent Adult Abuse.
• CPR: Basic Life Support for Healthcare Providers.

Responsibilities

• Conduct skilled home visits to assess patient condition and deliver care per plan.
• Complete initial and ongoing assessments and coordinate with physicians and care team.
• Provide hands-on skilled nursing care, patient education and caregiver training.
• Develop and revise care and discharge plans with measurable goals.
• Promote patient safety and independence while identifying risks.
• Coordinate care across interdisciplinary team and document orders.
• Maintain timely clinical documentation and OASIS assessments.
• Participate in case conferences, quality initiatives and peer review.
• Supervise home health aides and LPN staff as appropriate.
• Maintain productivity and support 24/7 care model.

The RN Case Manager - Home Health uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient's place of residence. The RN Case Manager - Home Health develops and oversees the home health plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient. The RN Case Manager - Home Health delivers patient care directed by the physician as established in the home health plan of care that is consistent with clinical best practices and results in high quality, improved outcomes and exceptional patient experience.

Responsibilities

• Conduct skilled home visits to assess patient condition, provide ongoing nursing evaluation, and deliver care aligned with individualized plans of care.
• Complete comprehensiveinitialand ongoing assessments,identifyingchanges in condition and coordinatingtimelyupdates with physicians and the care team.
• Provide hands-on skilled nursing care, patient education, and caregiver training on disease management, medications, safety, equipment use, and emergency preparedness.
• Develop, implement, and continuously revise individualized care and discharge plans with measurable goals, ensuring patient and family involvement in all decisions.
• Promote patient safety and independence while proactivelyidentifyingrisks for hospitalization and intervening to prevent avoidable complications.
• Coordinate care across the interdisciplinary team, communicate updates, and ensure physician orders are obtained, followed, and documented appropriately.
• Maintainaccurate,timelyclinical documentation, including visit notes, time tracking, verbal orders, and OASIS assessments in compliance with regulatory standards.
• Participate in case conferences, quality improvement initiatives, peer review, and interdisciplinary care coordination to support high-quality outcomes.
• Supervise home healthaidesand LPN staff asappropriate, ensuring safe, effective care delivery and proper documentation.
• Maintain productivity standards,participatein rotating evening/weekend coverage, and support a 24/7 care model while upholding professional communication and teamwork.

Qualifications

Education:

• Graduate of State Board approved program for Registered Nurses

Licensure/Certifications:

• Valid license as RN in state(s) where providing care.
• Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
• Mandatory Reporter: Child & Dependent Adult Abuse.
• CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

Minimum Experience:

• One year nursing experience caring for similar patient population as to be assigned.

Other:

• Use of usual and customary equipment used to perform essential functions of the position.
