Job summary

St. Camillus Campus is seeking a compassionate Registered Nurse to join our Home Health team as an RN Case Manager.

You’ll be the first point of contact for new clients, conduct assessments, develop care plans, and collaborate with an interdisciplinary team to deliver high-quality care in the patient’s home. You will conduct admission visits, evaluate eligibility, educate patients and families, coordinate care, and maintain compliant documentation.

Qualifications

• Wisconsin RN license required
• At least 1 year of Home Health nursing experience
• OASIS assessment experience preferred
• Current CPR certification required
• Valid Wisconsin driver''s license and reliable transportation

Responsibilities

• Conduct admission visits and complete OASIS assessments
• Evaluate patient eligibility for Home Health services
• Develop and coordinate personalized interdisciplinary care plans
• Educate patients and families on services, expectations, and care goals
• Collaborate with physicians and care team members to ensure seamless care transitions
• Maintain timely, accurate, and compliant documentation
• Participate in care coordination, scheduling activities, and on-call rotation
• Participate in an on-call rotation

What You'll Do

What You'll Bring

• Valid Wisconsin driver's license, reliable transportation, and required insurance coverage
