Job summary

St. Camillus in Wisconsin is seeking an RN Case Manager to coordinate, implement, and evaluate individualized hospice plans of care, using clinical judgment and compassionate communication to support clients and families.

The role requires current Wisconsin RN license, CPR certification, 1–2 years of clinical nursing experience (hospice preferred), strong assessment, documentation, and travel to client homes with a valid driver's license.

• Provide leadership, teaching, and support to hospice aides, licensed practical nurses, and other care team members.

What You’ll Do

• Perform timely initial evaluations and ongoing nursing assessments for hospice clients.

What You’ll Bring

• Current Wisconsin Registered Nurse license.

• Current CPR certification.
• One to two years of clinical nursing experience; hospice experience preferred.
• Strong assessment, communication, organization, and documentation skills.
• Ability to travel to client homes with a valid driver’s license, current vehicle liability insurance, and a driving record that meets St. Camillus standards.
