Base pay range

$68,000.00/yr - $85,000.00/yr

Job Summary: The Registered Nurse (RN) plans, organizes, and directs patient care with a focus on community health education and experience. The RN utilizes community resources to provide home and community-based care for individuals and families, ensuring their health needs are met.

Employment Status: Full-Time Monday- Friday 8am-4:00pm

We are committed to providing a transparent and equitable compensation process. The pay range for this position is $68,000 - $85,000 annually, determined based on factors such as experience, education, and qualifications. We encourage open discussions about compensation during the interview process to ensure clarity and fairness. While we may not be the highest-paying employer, we offer a strong team culture, robust PTO and a full range of benefits.

Key Responsibilities

  • Conduct comprehensive assessments of patients and families to identify needs.
  • Develop, implement, and revise individualized care plans.
  • Administer medications and treatments as prescribed.
  • Provide health education and counseling to patients and families.
  • Act as a Case Manager, coordinating care and ensuring continuity.
  • Maintain accurate clinical notes and communicate with physicians and health professionals.
  • Participate in on-call duties and interdisciplinary team meetings
