Mille Lacs Health System is hiring for an RN Case Manager - Home Care/Hospice - .8 FTE - Rotating On Call. Be among the first 25 applicants to apply for this opportunity.

Job Summary

Implement and facilitate client care plans, communicating with other disciplines to oversee client care effectively.

Key Responsibilities
  • Perform accurate new client admission assessments within 5 days. *
  • Maintain clear and accurate charting that reflects the client's status. *
  • Act as a client advocate and make appropriate resource referrals. *
  • Coordinate care with multiple/outside care teams. *
  • Demonstrate effective assessment and evaluation skills. *
  • Maintain professionalism and positive relationships with clients, families, and colleagues. *
  • Complete at least 5 visits and documentation daily, within an 8-hour workday unless otherwise approved. *
  • Practice infection control measures per standards. *
  • Report safety concerns and implement measures to promote safety. *
  • Identify safety and infection control needs. *
  • Ensure cost-effective client services and monitor supply use. *
  • Adhere to MLHS policies and demonstrate commitment to service excellence and teamwork. *
  • Participate in service evaluation and improvement activities. *
  • Maintain good attendance and punctuality. *
  • Engage in ongoing personal and professional development.
Qualifications
  • Graduate of an accredited nursing program with current RN licensure in Minnesota.
  • Knowledge of Minnesota Nurse Practice Act and Code of Ethics.
  • Previous experience preferred.
  • Valid driver's license and CPR/BLS certification required.
Additional Details

Salary: $34.23 - $51.35 per hour
