Job summary

Siouxland Community Health Center is seeking a dedicated RN Case Manager for the HIV Program in Iowa. The role is a full-time position, Monday through Friday, 8 AM to 5 PM, requiring strong nursing credentials and HIV care experience.

The candidate will manage nursing casework, coordinate referrals, monitor labs, and collaborate with local health departments to identify PLWHA as entry points into care. Flexibility for weekend/evening outreach is needed.

Qualifications

• RN licensure in state of Iowa or applicable to degree
• Current CPR and Mandatory Reporter certifications
• Basic computer and data entry skills required
• Strong knowledge of HIV and community resources
• Two years HIV care experience preferred or highly motivated
• Ability to work with diverse populations and professionals

Responsibilities

• Provide nursing case management and follow-up on referrals
• Monitor routine lab tests and immunization schedules
• Establish partnerships with medical practices and urgent care centers
• Collaborate with health departments to identify PLWHA as entry points
• Conduct outreach to high-risk populations and provide HIV education/testing
• Work flexible hours including weekends/evenings as needed
• Assist in overall operation of the early intervention program
• Maintain comprehensive patient records and adherence programs
