Job summary

UCLA Health in Los Angeles seeks a self-directed Advanced Practice Nurse to assess and coordinate care for a diverse patient population, collaborating with a multidisciplinary team to ensure safe, high-quality, cost-effective outcomes.

You will perform utilization review, discharge planning and care coordination, develop individualized care plans, and contribute to performance improvement across a world-renowned health system.

Qualifications

  • BSN or MSN (ASN accepted for current UCLA Health Nursing staff).
  • CA RN License and BLS certification.
  • Recent experience in case management, utilization management and discharge planning.
  • Minimum of three years of acute hospital experience; or the equivalent of education and experience.
  • Strong leadership abilities.
  • Systems planning and patient care management experience in a high-volume work environment.
  • Excellent communication, interpersonal, organizational and analytical skills.
  • Ability to work effectively and collaboratively with interdisciplinary teams.
  • Knowledge of a large university teaching hospitals.
  • Preferred: certified case manager and acute hospital experience as a case manager.

Responsibilities

Salary Range: $68.81 - $88.99 Hourly
