LanceSoft INC is seeking a designated Nurse Case Manager to deliver comprehensive case management services in an acute care setting. You will coordinate patient assessments, develop and implement individualized care plans, and oversee safe discharge planning in collaboration with a multidisciplinary team.

The role emphasizes evaluating medical necessity and ensuring compliance with regulatory and reimbursement standards to drive high-quality, cost-effective outcomes and continuity of care.

Qualifications

• RN license and accredited nursing degree required.
• Minimum 3 years in acute care nursing.
• At least 1 year in Case Management, Discharge Planning, or Utilization Review.
• Knowledge of regulatory and reimbursement standards.

Job Description

Position Summary:
Delivers comprehensive case management services within an acute care environment, ensuring seamless coordination across the healthcare continuum. Conducts thorough patient assessments and develops, implements, and evaluates individualized care plans in partnership with a multidisciplinary team. Oversees efficient and safe discharge planning while optimizing resource utilization. Evaluates medical necessity and ensures adherence to regulatory and reimbursement standards. Acts as a key clinical resource for patients, families, and care teams, driving high-quality, cost-effective outcomes and continuity of care.

Minimum Requirements:

• Graduate of an accredited nursing program
• Minimum three (3) years of acute care nursing experience
• At least one (1) year of experience in Case Management, Discharge Planning, or Utilization Review

Preferred Qualifications:

• Three (3) years of acute care case management experience
• Experience in Pediatrics, Emergency Department, or Utilization Review
• CCM or ACM certification
