LanceSoft is seeking an experienced Registered Nurse Case Manager to join a collaborative healthcare team in the United States. You will coordinate patient care, facilitate discharge planning, and connect patients with resources while working with interdisciplinary teams to ensure high-quality outcomes.

Qualifications

• Active RN license (or APRN) required.
• Minimum 2 years of recent Acute Care Case Management RN experience within the last year.
• Strong communication, organizational, and care coordination skills.

Responsibilities

• Coordinate patient care and discharge planning activities.

• Educate patients and families on treatment plans and resources.
• Collaborate with physicians, nurses, and multidisciplinary teams.

• Sign-On Bonus: Available on a case-by-case basis
