Job summary

Baptist Health in the United States is seeking an experienced RN Case Manager for Case Management. Qualifications

• BSN or combination of education and clinical experience may be considered in lieu of a bachelor’s degree.
• Current unrestricted licensure as a Registered Nurse in the state of Kentucky is required.
• Three years of job-related experience required.
• Case Management certification (ANCC, CCM or ACM-RN) or other approved Professional Certification is preferred upon hire and must be obtained within two years of hire.

Responsibilities

• Manage a designated caseload to coordinate and complete timely assessment, planning, implementation and evaluation of discharge plans and care transitions across the continuum of care.
• Ensure CMS and other external review agencies' requirements are met.
• Coordinate communication among the varied care providers and other essential parties internal and external to the health care organization.
• Identify patients who would benefit from case management services and manage assessment, planning, care delivery, monitoring and evaluation of care.
• For heart failure patients, focus on hospitalized patients and disease-specific case management to improve home outcomes and reduce readmissions.
