Job summary

CVS Health is seeking a full-time RN Case Manager in the Austin area to assess, plan, and coordinate care for members. The role requires an active RN license, at least 3 years of clinical experience, and strong computer skills, with bilingual English/Spanish preferred.

Qualifications

• Active RN license in their state of residence; multi-state/compact privileges preferred.
• At least 3 years of clinical practice experience.

Responsibilities

• Assess, plan, implement, and coordinate case management activities for members.
• Evaluate medical needs to facilitate the member’s overall wellness.
• Coordinate with Aetna programs and plans as needed.
• Use clinical tools and data review to assess needs and eligibility.
• Review prior claims and assess work capacity and restrictions.
• Refer to clinical resources and discuss cases in multidisciplinary conferences.
• Ensure compliance with regulatory and company policies.
• Engage members through interviews to determine health status and needs.
