Job summary

CornerStone Medical seeks an RN Case Manager for a contract role in Jeffersonville, IN. Compensation is $55/hr. Shift: M-F 8am-5pm; on-call rotating weekends. IN license required, compact accepted. Mileage reimbursement available at 0.43 per mile.

Responsibilities include comprehensive assessment of high-risk members, care plan coordination with providers, and efficient transitions to appropriate levels of care, with patient advocacy and documentation throughout.

Qualifications

• Associate's or Bachelor's degree in Nursing or related field.
• Current, unrestricted state RN license.
• Case Management certification.
• Must pass background check, drug screen, and have updated immunizations.

Responsibilities

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers to implement individualized, comprehensive care plans with specific interventions designed to engage the member.
• Address, identify, and continuously reassess cost-efficient, appropriate levels of care.
• Provide non-medical support to ensure compliance with treatment plans (e.g., housing and transportation).
• Coordinate care transitions as needed with physicians, social workers, discharge planners, and claims professionals to transition patients to appropriate level of care.
• Engage specialty resources as needed to achieve optimal resolution.
• Keep detailed records of clinical, functional, and fiscal outcomes during the management process.
• Identify opportunities for health promotion and illness prevention.

Primary Responsibilities

• Perform comprehensive assessment of high-risk members.
