At Brookestone Home Health & Hospice, we believe exceptional care begins with meaningful relationships. We're seeking a compassionate and skilled RN Case Manager to join our team and provide high-quality care to patients throughout Grand Island and the surrounding communities.

This role offers the opportunity to practice nursing in a way that truly makes a difference. As an RN Case Manager, you'll provide personalized care, coordinate services, advocate for patients, and support families through every stage of their healthcare journey. You'll enjoy the independence of caring for patients in their homes while benefiting from the support of an experienced interdisciplinary team.

What You'll Do
