Job summary

Brookestone Home Health & Hospice in Grand Island, NE, is seeking an RN Case Manager to deliver personalized home health and hospice care across Grand Island and surrounding communities.

You will assess patient needs, coordinate with a multidisciplinary team, and support families through the healthcare journey, while maintaining independence in home care within an experienced care team.

Qualifications

• Current RN license.
• Current CPR certification.
• Strong assessment, critical thinking, and clinical decision‑making skills.
• Excellent communication and care coordination abilities.
• Ability to work independently and manage multiple priorities.

Responsibilities

• Assess patient needs and develop individualized plans of care
• Coordinate services with physicians, therapists, caregivers, and other healthcare professionals
• Manage and monitor patient caseloads
• Provide education and support to patients and families
• Ensure quality outcomes through ongoing assessments and care planning
• Promote patient comfort, independence, and overall well being
• Deliver compassionate care through both Home Health and Hospice services
