Job summary

Aviator Home Health in Denton, TX is looking for a skilled Registered Nurse (RN) Case Manager to join our growing team. This full-time role involves comprehensive patient assessments, planning and adjusting care plans, and coordinating with physicians and family members.

You will deliver skilled nursing care, ensure infection control, complete documentation, and participate in on-call rotations. Texas RN license and at least one year of RN experience are required.

Qualifications

• Must be a Texas-licensed RN with at least 1 year of RN experience.
• Adult home health experience preferred.
• HomeCare HomeBase experience is a plus.
• OASIS experience is preferred.

Responsibilities

• Conduct initial and ongoing OASIS assessments when required.
• Develop or revise the plan of care and initiate nursing procedures.
• Provide skilled nursing care with appropriate infection control.
• Coordinate patient care and referrals; monitor outcomes and discharge planning.
• Communicate with patients, families, and health care professionals; document promptly.
• Supervise HHA/LVN when providing services.
• Participate in in-service programs and maintain competency.
• On-call duties as assigned.

Salary is based on many factors including but not limited to education, experience and location. Clinicians are in complete control of their earning potential. Once you meet your quota, you earn an additional $15 plus your regular rate as a bonus. Compensation is directly tied to effort; the more efficient you are the higher your earning potential.

Essential Functions of the Registered Nurse (RN) Case Manager

• Initial and ongoing comprehensive assessments of home health patient’s needs, including OASIS assessments at appropriate time points.
• Develop/initiate/revise the plan of care as necessary and initiate appropriate preventive and rehabilitative nursing procedures.
• Provide those services and/or treatments requiring substantial and specialized nursing skills. Provide skilled nursing care to patients utilizing infection control measures that protect both the staff and the patient (OSHA).
• Coordinate patient care, make necessary referrals, report to physician/other staff members changes in patient needs, evaluate outcomes of care and plan for discharge from service. Monitor assigned cases to ensure compliance with requirements of third party payor.
• Provide effective communication/counseling to patient/family, team members, and other health care professionals in meeting nursing and other needs.
• Complete appropriate documentation in a timely manner.
• Supervise and provide clinical direction to HHA/LVN providing services.
• Demonstrate commitment, professional growth by participating in in-service programs and competency.
• Promote Agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
