Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered RN license.
• 1+ year experience in case management, preferably in healthcare or hospital setting.
• Strong written and verbal communication to collaborate with medical teams, patients, and families.

Responsibilities

• Assess patients' medical history, medications, and psychosocial needs.
• Develop and implement individualized care plans with healthcare teams.
• Monitor progress and adjust plans as needed, coordinating changes with the team.
• Serve as the main point of contact for patients and providers to ensure seamless communication.
