Professional Case Management in Washington, DC is seeking RN-Case Managers to perform in-home nursing visits for former Nuclear Weapons Workers with chronic illnesses. You will assess health, ensure home safety, and coordinate care while building one-on-one relationships with clients.

Qualifikationen

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two years nursing experience including one year in home care or closely related field.
• One year of supervisory and/or case management experience preferred.
• Current CPR certification.

Aufgaben

• Provide direct care and case management of assigned clients.
• Plan, implement, and evaluate care provided; participate in care conferences.
• Collaborate with the regional director to identify issues and improve services.
• Coordinate staffing and scheduling within scope of practice.
• Maintain accurate clinical documentation and participate in quality initiatives.
