Job summary

Huntington Health is seeking a Case Manager to facilitate care delivery and timely discharge or transfer at our Pasadena, CA campus. The role supports patient flow and ensures care is appropriate and authorized, serving as a resource to the clinical team and payer networks.

RN with 3+ years in Trauma/General Surgery or Critical/Acute Care and strong discharge planning is preferred. This role requires flexible hours and full-time employment.

Qualifications

• Bachelor of Science in Nursing from an accredited college or university.
• Three years of RN experience in Trauma/General Surgery, Critical/Acute Care, or Emergency Care.
• Current unrestricted RN license issued by California BRN.
• Experience in case management and discharge planning is desirable.

Responsibilities

• Facilitate care delivery and timely discharge or transfer of patients.
• Manage patient flow to ensure care is appropriate, authorized and conducted in the most suitable setting.
• Provide clinical information and liaison support to payer networks and external case managers.
• Serve as internal resource to Revenue Cycle on clinical issues.
